Enrollment Request Form


For MCDB Courses ONLY*
(*may not be used for summer session courses)
This Form is only a request and does NOT guarantee enrollment.

You are eligible to use this form ONLY if you meet all three of the criteria below:

  1. You are unable to enroll through URSA due to pre-requisite problems and,
  2. It is during your URSA enrollment pass and,
  3. You are under the unit limit for your enrollment pass.

Additionally:

  • You must check URSA to confirm that you have been enrolled in the course.
  • We will not enroll you if you do not have the course prerequisites or if you do not meet the criteria listed above. For questions, contact the MCDB Undergraduate Counselor at undergradmcdb@lifesci.ucla.edu
  • Requests will be processed in the order received, and within 24-48 hours of receipt.
  • You will be contacted via e-mail if there is any problem with your request.
